Simpler paperwork, without leaving workers behind
Simplifying health care would change millions of administrative jobs. The HEALTH5 proposal plans for that change instead of counting those workers as instant savings.
A target of 500,000 permanent jobs
The proposal sets a planning target of about 500,000 permanent administrative employees across participating insurers and government agencies. They would handle the work that still needs people:
Members
Enrollment, customer help and appeals.
Payments
Claims and payment exceptions, auditing and risk allocation.
Protection
Fraud prevention, cybersecurity, compliance, data systems, quality control and oversight.
The 500,000 figure is a policy target, not a proven staffing requirement. A function-by-function workforce study would test whether it’s enough.
The Workforce Transition Corps
Suppose research finds 1.5 million people do this work today. If HEALTH5 needs 500,000, up to 1 million positions are affected. They wouldn’t all disappear at once.
Instead, a temporary Health Automation and Workforce Transition Corps would put those workers to work for perhaps two to five years building the automation that replaces today’s paperwork. They already know why claims fail, why payments get rejected, where eligibility breaks down, how appeals work, where fraud shows up and which decisions really need a human.
How the transition would work
- Simplify
- Retrain
- Automate
- Gain efficiency
- Build tech skills
- New jobs
CMS is already moving in this direction, using APIs and standardized data exchange to automate payer and provider processes (CMS-0057-F). Training would cover AI-assisted workflows, automated claims processing, health-data standards including APIs and FHIR, interoperability, cybersecurity, fraud analytics, process engineering, exception handling and system testing. The Department of Labor’s AI skills website and AI-focused Registered Apprenticeships, launched April 29, 2026 with health care training included, are a ready starting point (U.S. Department of Labor).
The Corps would shrink through retraining, reassignment, normal attrition and retirement, not mass layoffs.
Why we don’t promise a number
No one has yet counted the current health-administration workforce across federal, state, local and private employers function by function. Until that count exists, claiming HEALTH5 would “eliminate a million jobs” would be guesswork. The honest plan sets the 500,000 target, measures today’s workforce, and treats the difference as a transition workforce.
The 500,000 target and the 1.5 million example are planning assumptions for further study, not government workforce forecasts.
